CAR T-Cell Therapy for Leukemia

Sheba Medical Center provides CAR T-cell therapy for leukemia, an advanced form of immunotherapy for both adults and children.

This treatment involves modifying a patient’s own T-cells to recognize and target cancer cells. Sheba offers access to CAR-T therapies, including programs with point-of-care manufacturing capabilities, allowing some treatments to be produced locally.

*Important: Patients accepted into Sheba’s point-of-care CAR-T program benefit from in-house cell manufacturing at Sheba. However, pediatric patients with leukemia are currently only eligible for commercially available CAR-T therapies.

How Does The Therapy Work for Leukemia?

CAR T treatment for leukemia is an innovative process that uses your own immune cells to seek and destroy cancerous cells.

An amount of blood is taken from the patient and the T-cells, a type of immune cell, are separated out from the rest of the blood in a special laboratory. The T-cells are then genetically engineered to cause them to produce chimeric antigen receptors (CARs). The CARs will allow the T-cells to recognize tumor cells, targeting and destroying the tumor.

After some blood is taken from the patient, the T-cells, a type of immune cell, are removed from the sample. Using advanced genetic engineering techniques, the T-cells are modified, causing them to create chimeric antigen receptors (CARs). These CARs program the T-cells to find and destroy cancer cells when injected back into the patient.

The patient receives chemotherapy while the modified T-cells grow in our laboratory. The chemotherapy dampens the patient’s immune system, permitting the CAR T-cells to do a better job of eliminating malignant cells.

What are the Possible Side Effects from CAR T-Cell Therapy?

CAR T-cells are associated with significant but reversible side effects in 40-60% of patients treated.

These include a flu-like syndrome with fever and shortness of breath. This syndrome may lead to low blood pressure, chills, and more. Neurologic side effects, like a headache and seizures, have been reported. All these effects are reversible, but may be significant and require treatment in the ICU.

Who is a Candidate for CAR T-Cell Therapy for Leukemia?

The therapy is only provided to patients who have undergone at least two previous leukemia treatments.
